Once we have scheduled a consultation, if you would like me to review any information prior to that meeting, then please use these links.
Each link opens a secure, encrypted page where you can enter information into a form (note that http:// in the web address bar changes to https:// to indicate a secure link).
When you click the submit button, the information is transmitted to a secure server hosted by Adobe Forms Central which I can then access. Just close the window after pressing the submit button.
Please feel free to send as much or as little information as you want. The first form: "Basic Demographics" is particularly helpful but feel free to not use it if you prefer.
